Introduction: Pressure injuries (PIs) represent critical challenges in Long-Term Care Facilities (LTCFs). This study analyzes the impact of care bundles combined with nutritional optimization on PI prevention. Methods: A narrative synthesis was conducted based on an LTCF in São Paulo serving 31 older adults. In 2025, following care delivery failures, a protocol was implemented focusing on three pillars: advanced skin care, personalized nutritional support, and mobilization/pressure redistribution bundles, benchmarked against international guidelines. Results: The facility implemented 83% of the essential recommendations. The integration of risk assessment, support surfaces, and specialized supplementation (protein and micronutrients) resulted in a 0% PI incidence rate. Correlated evidence confirms that such interventions significantly reduce risk. Conclusion: The adoption of integrated protocols and active clinical governance is highly effective. This study reinforces that optimized nutrition, combined with mechanical management, is a central and replicable intervention for eliminating PI incidence in institutionalized older adults.
